Insurance Information

At the Practice of  Dr. Burns we try to provide you with the finest care and the most convenient financial options. To accomplish this, we work with you to maximize your insurance reimbursement for covered procedures. Before your appointment, you must verify that we are your insurance provider, whether by contacting us or your insurance provider. If you have any problems or questions, please ask our staff. Please be advised that we do not submit to secondary insurance or plans for which we are not a provider. You can obtain an itemized receipt at the time of service, which you can submit to your insurance provider. Please call if you have any questions or concerns regarding your initial visit.

Call our office with your insurance information before your appointment so we may provide you with a quote based on your coverage.

If you fail to provide your insurance information 24 hours before your appointment, we may not have enough time to verify your coverage. Without verified coverage at the time of appointment, you will be responsible for the full cost of treatment without insurance calculation.\

We will send you a monthly statement. Most insurance companies will respond within four to six weeks.

Please call our office if your statement does not reflect your insurance company’s payments during that period. You are responsible for any remaining balance after your insurance has paid its portion. Your prompt remittance is appreciated.
